Centers listed by capability, then region as a secondary attribute. For severe, multiplanar adult post-clubfoot deformity — especially with chronic wounds, compromised soft tissue, or threatened limb — the operating team must combine complex reconstruction with limb-salvage discipline.

Capability to look for

  • Complex adult foot & ankle reconstruction: fellowship-trained orthopaedic foot & ankle surgeons with documented revision / adult residual-clubfoot volume. Prior soft-tissue release leaves scarred anatomy that standard approaches may not fit.
  • Limb-salvage / preservation: ortho + vascular + plastic/reconstructive + infectious disease working in one coordinated program. Salvage decisions (reconstruct vs. amputate) benefit from a single decision-making team.
  • Advanced techniques: external fixation (TSF / Ilizarov), staged reconstruction, free-tissue transfer, and — where indicated — ankle arthrodesis or arthroplasty.

When to consider a high-volume national program

  • Multiple prior failed reconstructive attempts.
  • Active or recurrent osteomyelitis.
  • Non-healing wound >6 months despite optimal care.
  • Severe multiplanar deformity requiring staged reconstruction or external fixation.

In these scenarios, travel to a program that can assemble the full multidisciplinary team often outweighs staying with a local option that cannot.
